Common questions

How is Persimmony different from an EHR?

EHRs were built to document clinical encounters inside a clinic. Public health and human services work happens everywhere else — in homes, schools, shelters, and across multiple programs that each have their own funding rules, reporting requirements, and definitions of a "case." Persimmony is purpose-built for that work. Instead of forcing program staff to bend their workflows to a clinical system, Persimmony manages clients across programs, tracks time and outcomes against grants and FFP, and produces the reports your funders actually ask for.

Can Persimmony support multiple programs and funding streams in one system?

Yes. One client, one record, every program. Whether a family is enrolled in home visiting, WIC, TB follow-up, and a community health initiative simultaneously, Persimmony links the work without duplicating data entry. Each program keeps its own forms, eligibility logic, and reporting — but staff and supervisors see the whole picture across the family's full journey.

What programs does Persimmony support?

Persimmony is built to flex around the programs your department runs, not the other way around. Customers use it across maternal and child health (Nurse-Family Partnership, Healthy Families America, Help Me Grow), behavioral health, harm reduction, communicable disease investigation (HIV, STD, TB), home visiting, community health worker programs, and care coordination for older adults — among many others. If your program isn't named here, it almost certainly fits the same patterns. Because Persimmony is configurable rather than program-specific, departments stand up new programs in the same system as priorities and funding shift.

Does Persimmony replace or work alongside an EHR?

Both, depending on what your department needs. Many of our customers run Persimmony as their system of record for community-based programs while a clinical EHR handles encounters inside a clinic. When that's the right setup, Persimmony exchanges the data that matters — referrals, shared client demographics, care plans — so neither team is re-entering information. For departments without a clinical footprint, Persimmony operates as the complete system. The point is that public health programs shouldn't be forced into a clinical tool just because one exists nearby.

How does Persimmony make sure nothing falls through the cracks?

Follow-through is what separates good case management from great case management — and it's where most systems quietly fail. In Persimmony, every referral, follow-up, screening, and care plan task is tracked against the client, not the nurse or case worker who opened it. When a referral goes out, the system tracks whether it was received, accepted, and acted on, and flags the ones that stall. When a family is handed off between programs or staff, the next person sees the full history, what's pending, and what's due — without a re-intake. Supervisors see what's open, what's overdue, and which families are at risk across every program touching them. The result: fewer missed follow-ups, clearer accountability, and a record that reflects the full arc of care a family received — not five disconnected ones.
