Persimmony is now the trusted partner for 10 First 5 Commissions, their evaluators, and the programs they fund. In addition to partnering with First 5 San Diego and First 5 Monterey, Persimmony provides database solutions and data management continuity for First 5 San Bernardino, WRMA (First 5 Sacramento), First 5 Contra Costa, First 5 Fresno, First 5 Humboldt, First 5 Marin, First 5 Solano, and First 5 Yolo.
Persimmony has been assisting First 5 County Commissions throughout California to measure the impact of Proposition 10 funded programs using Persimmony’s First 5 Database, the leading Internet-based application and online database for outcomes measurement with large and diverse funding initiatives. The funded programs using Persimmony collect and store service and outcomes data using Persimmony’s easy-to-use online interface. Commission staff and their Evaluators then use the data to measure the impact the funded programs are having on the children and families they serve.

First 5 was created after voters passed Proposition 10 in November 1998, adding a 50 cents-per-pack tax on cigarettes to fund education, health, childcare and other programs for expectant parents and children up to age five. Proposition 10 was designed to address the lack of public funding and support for early childhood development because the emotional, physical, social and developmental environments to which children are exposed. These environments can have a profound impact on their ability to reach their greatest potential in school and to become productive members of society.

Persimmony Developing Home Visiting Data System (HVDS) that Spans Public and Private Service Partners

Persimmony’s Home Visiting Data System (HVDS) provides solution for four home visiting programs: Prenatal Care Guidance (PCG), Public Health Nursing (PHN), Medically Vulnerable Infant Program (MVIP), and Welcome Home Baby (WHB).

ALISO VIEJO, CA September 12, 2008 — Michael Kogus, President and CTO of Persimmony International, announced today that Persimmony has been awarded a database development and management contract with four home visiting programs in Contra Costa County: Prenatal Care Guidance (PCG), Public Health Nursing (PHN), Medically Vulnerable Infant Program (MVIP), and Welcome Home Baby (WHB). The custom data system is meant to assist these programs and their home visiting personnel capture and manage case data, increase their ability to access drawn down Targeted Case Management (TCM) funding, and develop data sets that can be used to measure effectiveness, outcomes, and gap analysis within Contra Costa’s Home Visiting program.

Persimmony’s HVDS solution for Contra Costa will:

  • Provide home visitors a mobile, up-to-date electronic case record by which they may make the best patient care decisions possible and make effective referrals in the field.
  • Establish a common home visiting client electronic record format for use by home visitors in medical, mental health, social support or other programs
  • Ensure client confidentiality by the establishment of, and adherence to confidentiality agreements and protocols.
  • Allow for interface with a range of programs to allow for the most comprehensive and integrated approach possible to serving clients.
  • Provide program managers with information regarding the flow of cases through their programs.
  • Support evaluation of the home visiting system as a whole to determine the impact of home visiting on families and within communities.
  • Provide a mechanism and the tools to identify and direct best practices and activities for working with children in the home visiting system and to know how well the system functioning within various aspects of the programs (e.g. screenings, referrals, follow-ups, etc.).
  • Support the administration of targeted case management (TCM) and other funding sources in public health nursing and community-based organizations.

All four Contra Costa home visiting programs have devoted significant time to develop the system specifications that meets their own program needs as well as collaborative program needs. With over 1,400 data fields, innovative elements of the new Persimmony HVDS will include joint program referral and enrollment, program-specific medical and psycho-social assessments (over 30 different assessments), case planning, follow-up, and case closure.
